What insurance providers does Free Body Physical Therapy accept?

We accept Blue Cross Blue Shield PPO/EPO, Aetna, MedRisk, Corvel, Medicare, Worker’s Comp, Auto. Please contact us if your health insurance is not listed here or to inquire about your benefits. We also accept self-pay patients via cash/credit card.

What languages do we speak?
Dr. Guillen is fluent in  Polish, English and Spanish.
What is the average length of an appointment?
45-60 minutes.
What should I wear to my first appointment?
For low back, hip, knee, ankle injuries please bring shorts. For neck or shoulder injuries wear clothing that allows the shoulder blades to be visible.
Are there any new patient forms I need to complete prior to my first appointment?

The front desk staff will forward you the forms once you register as a patient.

How often will I have to complete exercises at home?

Each exercise program is custom tailored to your injury. The goal is to make your home program fit your busy life.

How is Free Body Physical Therapy different from your standard chain physical therapy practice?

We provide one-on-one care without the use of rehab aides or Physical Therapy Assistants. We try to empower you to understand your injury so you don’t need to come 2-3x/week for months. We do not use modalities such as Ultrasound & Electrical Stimulation as research shows this doesn’t accomplish much.
